Passport Information


FOR PASSPORTS THE FOLLOWING INFORMATION IS REQUIRED:

You will need a current application form properly completed. Forms must be filled out in black ink only.

Forms can be obtained from the County Clerk’s Office or by visiting the website at travel.state.gov. (Form DS-11) You can fill the form out and bring it in. Please do not sign the form until requested to do so.

Proof of Citizenship: A certified birth certificate must include your given name and surname, date and place of birth, date the birth record was filed, and the seal or other certification of the official custodian of such records.

Proof of Identity: A valid driver’s license or pictured ID.

Two photographs, of you alone, sufficiently recent and 2 x 2 inches in size. Your picture can be taken in the County Clerk’s Office for a fee of $10.00.

A $25.00 per applicant processing fee to the County Clerk’s Office in cash, check, or money order.

A $110.00 check or money order only made out to U. S. Department of State for applicants 16 years or older; if you are 15 years or younger, the fee is $80.00 plus the $25.00 processing fee.

Office Hours:

Applications for passports are processed Monday – Friday between the hours of 8:00 a.m. – 4:00 p.m.

All applicants must appear in person.
